Advance Directive Limiting Care upon admission
Functional Dependence (needs equipment or any person for ADLs)
CRF on dialysis, PD, HF, or hemodiafiltration

Stroke/CVA WITH DEFICIT (any deficit, but must refer to the fact they have any deficit even impaired memory)

Alcohol use disorder
